Interesting how people try and measure up to the standard of revival themselves, making the whole thing smell of human effort a bit too much.

The precursor to the list of 5 should be:

A hungry people, crying out to God for change.

Only God can revive; we just need to arrive at the point of recognition that our human effort is futile at best, and cry out to him from that place of mortal helplessness.

If we REALLY want it, God will do it. Not us.

I think we often use the term 'revival' in the wrong setting - to mean people coming to faith. I remember someone pointing out to me that you can't revive something that wasn't there in the first place, and therefore, revival is about the church ('Send revival, start with me' always springs to mind).

I just looked up revival in the dictionary and thesaurus and love some of the words and descriptions that it comes up with...
